Planning a loved one's funeral can be very stressful especially while trying to navigate through your grief. Our Lady of the Visitation Catholic Church is here to help and support families through these difficult times. As part of this support, we have developed the form below to assist families in planning a funeral. This is form designed for family use only. All funeral homes will need to contact our Funeral Coordinator directly at the parish office during normal office hours. Any funeral request forms submitted by a funeral home will not be honored.
